Documentation is an essential part of patient care. When documentation is incomplete, your patient’s safety and provider liability are at risk. If the patient claims they have been harmed, one of the first steps taken is a review of your documentation in the medical record. Could you defend yourself against medical negligence with your medical record documentation alone?
